You will need to fill out a new patient form, as well as provide your dental insurance card (if you have insurance). If you’re unable to fill out the form in advance, please arrive 15 minutes prior to your appointment.

We believe in collaborative, comprehensive care. We will be reviewing your medical and dental history paperwork, taking a complete set of x-rays (if required), and performing a thorough and complete oral health evaluation. We listen to understand your oral health goals and concerns. Based on our findings, we create treatment options and develop a plan you can feel confident in.
